First Session: Your first session will include a fifteen minute complimentary
intake that will allow us time to discuss the goals you hope to achieve from your session. We will review your client intake
and health history form, so that we may address certain conditions or answer any questions you may have for me.
Payment Structure: Payment is due when services are rendered. I accept personal checks and
cash. Sessions are as follows: 30 minutes $30, 60 minutes $60, and 90 minutes $90. I do require 24 hours advance notice if
you need to cancel your appointment. Less than 24 hours notice may result in a cancellation fee of $20. However, I do understand
that situations and emergencies arise that may make it difficult for you to give adequate notice of your cancellation, and
every effort will be make to take these situations into consideration when assessing cancellation fees.
If you arrive
late for your appointment and would still like to receive your massage at that time, the length of time for your session may
be shortened based on the time you arrived, and full payment will still be due. If you arrive more than 15 minutes late for
your appointment, you may be required to reschedule and may be charged a $20 rescheduling fee. This will be at the sole discretion
of the massage therapist.
Privacy: During the
massage your modesty will be respected at all times by keeping your body covered by the sheet. I will only expose the body
part on which I will be working.
Confidentiality:
Personal and medical information, along with anything you may disclose to me during a session, are strictly confidential.
I will not release this information to anyone, without your written permission or as requested by court of law.
Behavior: Any illicit or sexually suggestive remarks or advances made by the client
will result in immediate termination of the session. In such cases, client will be liable for payment in full of scheduled
appointment.
